Remote Patient Monitoring Program (Blood Pressure & Weight Scale)

What is the Remote Patient Monitoring Program?

Remote Patient Monitoring (RPM) involves using a cellular-connected blood pressure or weight scale device to record personal health and medical data. This helps your provider monitor your vitals between clinic visits and can be used to monitor long-term and new conditions. The continuous tracking is helpful for patients with ongoing care needs, such as those with diabetes, heart conditions, asthma, and hypertension. Creating the daily habit of checking your blood pressure and weight can help you be more successful at managing your medical conditions, as well as seeing the impact lifestyle choices have on your overall health.

Will my doctor have access to my readings?

Yes, your provider will have access to the Nudj portal via a computer or the Nudj Health app. Blood pressure and weight readings will be transmitted to the Nudj Health team for review. The Nudj RPM team reviews hundreds of these readings daily. Your doctor has already given the Nudj Health team instructions on what measurements are too high or too low. If the Nudj Health team sees an abnormal measurement, the team will follow your doctor’s instructions which can include calling you to check on your status and/or notifying your doctor and his or her staff.

Why did I not receive a weight scale?

Not all insurance plans cover Remote Patient Monitoring (RPM) services, which is required to receive a weight scale. Please confirm with your Registered Dietitian to see if this is the case. If you would like to pay for the weight scale and have your measurements automatically uploaded to the Nudj Health app, please discuss this with your Registered Dietitian.

How often should I take my blood pressure?

We recommend taking your blood pressure at least twice a day: once in the morning right after you wake up and have used the restroom and once in the evening before you go to bed. Taking your blood pressure at the same time every day is helpful to identify trends.

How often should I weigh myself?

Nutrition patients: Weigh yourself daily; first thing in the morning after using the restroom and before eating or drinking anything.

Heart Failure patients: Weigh yourself daily; first thing in the morning after using the restroom and before eating or drinking anything. Weigh yourself at the same time each day, use the same scale, and wear the same type of clothing. If your weight has changed more than 2-3 lbs in a day, or more than 5 lbs in a week, please contact your provider.

How are the blood pressure readings transmitted?

Your device connects to AT&T cell towers. No Wi-Fi connection is required. When the device is done transmitting your reading via cell towers, the device will automatically turn off. When it turns off this is evidence that your readings have been successfully transmitted.

How often are the blood pressure measurements reviewed, and who is reviewing them?

The Nudj RPM team reviews transmitted blood pressure readings within 24 hours of receipt on regular business days, which are Monday-Friday, except for some holidays. Transmissions sent on a Saturday, Sunday, or any holidays will be reviewed the following business day.

Does my provider have access to my blood pressure measurements?

Yes, your provider can access your Nudj profile and view your readings through the Nudj portal or in the Nudj Health app.

Do I have access to my blood pressure measurements? How do I access them?

Yes, you can view your measurements by logging into your Nudj account through a web browser or the Nudj Health mobile app. If you have not created an account profile, you’ll need to set that up first.

What happens if I have critically high or low blood pressure?

If a reading from your device has been transmitted and is critically low (below 75) or critically high (above 200), the Nudj team will notify your referring provider during regular business hours.

How do I know Nudj is receiving my transmissions?

You can verify device transmissions by logging into your Nudj profile via the web app or mobile app. If we don’t receive blood pressure readings for two or more days, you will receive an automated message reminding you to take your blood pressure.

Can I travel with my blood pressure monitor?

Yes, you can travel with your device as long as it is within the U.S. or a U.S. territory. If you plan on traveling, please be sure to let us know so we can shut off automated reminder notifications during your travel dates.

How do I set up my blood pressure and/or weight scale?

Insert the batteries and place the cuff on your arm to take your first reading. The device will automatically send the transmission straight to our platform. We also provide a complimentary Quick Start class that can provide additional set up assistance. This Quick Start class is also a great way to learn the basics of the device, confirm real-time receipt of transmissions, and learn more about the RPM class offerings and programming available to you.

Which arm should I place the cuff on?

If this is your first time using the blood pressure cuff, please take an initial blood pressure reading on both arms. Determine the higher reading and use that arm only for future readings unless otherwise instructed. If you have a port in place, arteriovenous fistula, or lymphedema, please use the arm without the aforementioned.

Important: Please notify your doctor on your next visit if your blood pressure was more than 10 points higher in one of the arms (note which arm had the 10+ reading).

What should I do if my device shows an error code?

How do I sign up for RPM classes?

What CPT codes are billed for RPM?

Remote Patient Monitoring is billed to your health insurance with the following CPT codes:

  • 99453: Initial patient set-up.
  • 99454: Remote patient monitoring – Data transmission.
  • 99457: Time spent managing Blood Pressure transmission.
  • 99458: Additional time spent managing Blood Pressure transmission.

Why does the billing date not match my appointment date?

All billing codes for Remote Patient Monitoring are submitted at the end of a calendar month and not on the date of any appointments or the dates you took your blood pressure or measured your weight. Exception: CPT 99453 is the only code billed at the end of the billing cycle.

Who will bill my insurance?

Your referring healthcare provider will be the one billing your insurance, not Nudj Health.

Collaborative Care (CoCM)

What is Collaborative Care?

Collaborative Care uses a research-backed approach to healthcare to improve your health. Your care is delivered by a team of experts in many different areas and includes your ordering physician, your lifestyle medicine physician, care management staff, and a psychiatrist consultant. The care management staff may include nurses, clinical social workers, health coaches, registered dietitians, exercise physiologists, yoga instructors, and fitness specialists. Most chronic diseases develop slowly over time because of lifestyle and behavior choices. Our consulting psychiatrists are experts in human behavior and emotions and helps your care team support you in reaching your goals.

Can I choose to focus on two lifestyle areas at the same time?

Unfortunately, no. Our program is designed to help you focus on one lifestyle area at a time to optimize your ability to develop healthy habits that can sustain a lifetime. After graduating from one lifestyle area, you will be able to choose another area. You will experience the most health improvements when you focus on one area at a time.

How many appointments do I have to attend per month?

Each month, you will meet with your specialist for a one-on-one appointment and attend two group classes for education and experiential learning. You will also have access to Nudj Academy classes which you can take throughout the month to enhance your health goals.

How long is the program?

Each lifestyle area is designed to take 16 weeks to complete when following the program prescription of 2 group classes and 1 one-on-one per month. If you miss one or more classes/appointments, it may take you longer than the 16 weeks to complete the program. You must attend all classes and appointments as prescribed in order to optimize your outcomes and be considered as complete.

What are the qualifications of the Nudj Collaborative Care team?

The Collaborative Care team includes your ordering physician, care management staff, and a psychiatrist consultant. The care management staff may include nurses, licensed clinical social workers, health coaches, registered dietitians, 200 hr. and 500 hr. registered yoga teachers, exercise physiologists, and fitness specialists.

What is the cost of the program?

Most insurance plans cover the Nudj program. Our Patient Navigator team will discuss your insurance coverage during your enrollment call.

Important: If you have not yet met your deductible for the year, you will be responsible for the out-of-pocket cost until you meet the remaining amount. After meeting the deductible, you will only be responsible for the co-pay and/or co-insurance. If your co-pay and/or co-insurance is high, you can also ask about our direct pay program where you can opt out of using your insurance. In some cases, this may be a less expensive option.

What should I do if I need to reschedule my appointment?

You can contact your health coach or care manager via email, text or phone to reschedule appointments and/or classes. They will work with you to get you rescheduled for the current month to ensure you meet the program prescription and complete the program in the allotted 16 weeks.

Important: You must cancel an appointment at least 24hrs in advance to avoid incurring the $25 no show fee. Please review the No Show Policy section for more details.

What is the No Show Policy?

Appointments must be canceled at least 24hrs prior to your appointment/class time. If canceled less than 24hrs in advance, a $25 fee will be charged. Your appointment reminders and Welcome Email will include reminders of the no-show policy. This policy is designed to help support your success in the program and to respect the time and efforts of our staff and other patients. You can reschedule or cancel at any time without being charged the fee as long as the request was received 24hrs prior to your originally scheduled appointment and/or group class.

Why does the Explanation of Benefits (EOB) I received from my insurance company list Collaborative Care as a psychiatric service?

Your Explanation of Benefits (EOB) will list Collaborative Care as psychiatric services while you are in any of the following programs: Wellness/Risk Reduction Program, Cardiac Complete, Cardiometabolic Change. This does not mean you have been diagnosed with a psychiatric illness. Instead, it refers to the supervision and guidance provided by our team of psychiatric consultants to the lifestyle program. As behavior change experts, they work behind the scenes with your Nudj care team to guide you on your path to great health. It takes a village of support when making lifestyle changes this collaborative team is your village!

What CPT codes are billed for Collaborative Care?

Collaborative Care is billed to your health insurance with the following CPT codes:

  • 99492: Initial Collaborative Care Management appointment
  • 99494: Additional 30 minutes for initial appointment
  • 99493: Subsequent Collaborative Care Management appointment.
  • 99494: Additional 30 minutes for subsequent Collaborative Care Management appointment
  • G2214: Less than 30 minutes – Initial or subsequent Collaborative Care Management

Cardiac Complete

What is Cardiac Complete?

Cardiac Complete is a virtual lifestyle medicine program designed to improve cardiometabolic health by targeting the following areas: cardiovascular fitness, resistance training, stress management, nutrition, social connection and mindful movement. Patients will have the opportunity to engage with content experts in each of these areas to improve their health by attending virtual sessions with specialized Nudj clinical staff. This program is offered as part of the Collaborative Care Model described in the Collaborative Care section.

Cardiac Complete utilizes evidence-based interventions to reduce the risk of developing chronic conditions such as high blood pressure, high cholesterol, obesity, diabetes, and more, while also reducing the severity of current chronic conditions and improve associated symptoms for improved quality of life.

What kind of benefits can I expect from the program?

Cardiac Complete strives to prevent and reverse chronic diseases through personalized health coaching and participation in virtual classes empowered by Nudj specialists and peer learning. Class topics range from fitness, stress management, mindful movement, and nutrition.

Patients who followed the program as prescribed showed improved health such as:

  • 74% reduction in A1c
  • Averaged a 13.3 lb weight loss
  • Decrease in stenosis after one year
  • 37.2% reduction in LDL cholesterol (referred to as “bad” cholesterol)
  • Decrease in anginal episodes
  • Of participants considered, 21% of patients were able to decrease medications used to manage diabetes

How many appointments do I have to attend per month?

You will experience the best results if you follow the program as prescribed, which is an average of 8-10 classes per month and a one-on-one appointment with your Nudj specialist. Your monthly class schedule will include a combination of fitness, nutrition, and stress management classes. During your monthly appointment with your Nudj specialist you’ll work together to build your class schedule for the following month. Sticking to an average of 2 classes per week will not only help you stay on track to complete the program in the recommended amount of time but allow you to experience optimal outcomes as well. Length of class time will vary, with fitness classes being an average of 60 minutes and educational classes being an average of 45-60 minutes.

Virtual Cardiac Rehabilitation

What is Virtual Cardiac Rehabilitation?

Virtual Cardiac Rehabilitation is a research-backed program proven to improve the health outcomes of people with certain types of cardiac disease. The program can now be provided as part of your physician’s care from the comfort of your home. The program lasts 18 weeks (about four months) where you will attend an average of 20 classes per month focusing on exercise, nutrition, and stress management. Flexible schedule offerings are available to allow you to fit this program into your life. Once your physician prescribes this program, you will meet with your intake specialist to create your treatment plan. Your start date will be dependent on when your physician clears you for exercise. Cardiac Rehabilitation will occur in a virtual group setting through a telehealth platform. Your care team will include nurses, cardiac experts, exercise physiologists, registered dietitians, 200 hr and 500 hr registered yoga teachers, clinical social workers, and case managers. You will also receive a blood pressure device and pulse oximeter to monitor your vital signs daily as well as before, during, and after each exercise session. You will be instructed on how to use these devices during orientation and your individual appointments with your specialists.

What CPT codes are billed for Virtual Cardiac Rehabilitation?

The Nudj Virtual Cardiac Rehabilitation is billed to your health insurance as Intensive Cardiac Rehabilitation with the following CPT codes:

  • G0422: Intensive Cardiac Rehabilitation with exercise
  • G0423: Intensive Cardiac Rehabilitation without exercise

The program lasts 18 weeks (about 4 months) with 2 fitness-based group classes per week and 1 individual appointment with a Nudj specialist per month. Flexible schedule offerings are available to allow you to fit this program into your life. Once your physician prescribes the program, you will meet with your intake specialist to create your customized schedule and program plan. Your start date will be dependent on when your physician clears you for exercise. Pulmonary Rehabilitation will take place in a virtual group setting through a telehealth platform. Your care team will include nurses, pulmonary fitness experts, registered dietitians, 200 hr and 500 hr registered yoga teachers, and case managers. You will also receive a pulse oximeter to monitor your vital signs daily as well as before, during, and after each exercise session. You will be instructed on how to use the pulse oximeter during orientation and your individual appointments so you can monitor your health and improve your ability to manage your condition.

Chronic Care Management

What is Chronic Care Management?

Chronic Care Management is a program designed to help those managing two or more chronic conditions. If you choose treatment with Chronic Care Management, you are declining Lifestyle Medicine treatment with Collaborative Care. The Chronic Care Management program includes:

  • A personalized and comprehensive care plan, created and shared with you, to help you manage your diseases and any needs impacting your health
  • Coordination with outside resources as appropriate
  • Assistance with medication compliance and symptom management
  • Monthly check-ins with your care manager
  • Remote monitoring of your blood pressure and/or weight, if prescribed by your referring doctor

  • 99487: Service 60 minutes – Chronic Care Management
